Dead: Damian Lackhan

It is yet unclear what resulted in the gate’s malfunction, but according to a source at the Ministry of Social Protection, Lackhan was reportedly assisting three others at the time, after being called over to help.

The Ministry of Social Protection’s Department of Labour, after recording 245 workplace accidents up until August 2016, announced that very month that it would be working with select employers to train safety and health committees as a means of reducing workplace accidents.

An official within the ministry had revealed that as many as 62 companies would be invited to partner with the Ministry, those being companies that have in the past reported both fatal and non-fatal accident.

The initiative was expected to be rolled out last September.
